Family Support Worker

Full Time Position

Community Living Walkerton and District is a not-for-profit organization that supports people with developmental and intellectual disabilities to live meaningful, active lifestyles in their community. You will be joining a dedicated and committed team whose mission is to nurture inclusion and promote people’s full participation as valued and contributing citizens.


Position Summary
Reporting to the Manager – Family Support Services, the Family Support Worker is responsible for providing family-directed supports that assist families throughout Bruce and Grey Counties to connect with community resources and developmental services.


Employment Details
Position: Full-Time Family Support Worker
Hours of Work: Monday to Friday, 8:30 a.m. to 4:30 p.m.


Responsibilities
• Assist families in accessing community resources and funding mechanisms.
• Support youth and families through transitions within elementary school, high school, and life after high school.
• Participate in transition planning.
• Consult with professionals and coordinate services as requested by families.
• Facilitate the development of support networks.
• Assist adults and families in coordinating and planning the use of Passport funding.
• Maintain current knowledge of available community resources and services.


Qualifications
• Demonstrated commitment to the mission, vision, and values of Community Living Walkerton and District.
• Post-secondary education in developmental or social services and related experience supporting persons with developmental disabilities.
• Familiarity with community resources in Bruce and Grey Counties.
• Strong communication and interpersonal skills.
• Computer proficiency, including Microsoft Office and electronic documentation systems.
• Valid First Aid/CPR certification or willingness to obtain.
• Valid driver’s license and willingness to travel throughout Bruce and Grey Counties.

Part -Time Support Worker

The successful applicant will provide support to adults in all aspects of daily living in their home and community.

Wage Range: $22.97 to $24.97 per hour day rate; $19.91 per hour night rate.

Position Summary:   This position provides scheduled support to people in receipt of ongoing service, encouraging personal choice, meeting needs in all areas of daily living, creating an atmosphere of caring and respect to facilitate a positive home life and an active life style.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

  1. DIRECT SERVICE

 (A) Attention to the physical needs of people through assistance with:

  • personal hygiene and appearance
  • nutrition
  • medical needs and safety
  • supervision and support of individuals in routines of daily living

(B)       Interaction with people who request support by:

  • development of a positive relationship between Support Worker and the individual
  • ensuring emotional and social well-being
  • advocating to ensure that rights, dignity, and personal desires are respected
  • promote a sense of belonging, self-worth, self-expression and security for each individual

(C)      Attention to interactive activities through:

  • encouraging social, personal and family participation as well as community involvement.

(D)      Attention to growth challenges through:

  • providing continued opportunities for the development of productive informed decision-making and independence of people supported.
  1. ADMINISTRATION

(E)       Writing responsibilities:

  • accurate recording of daily expenditures and activities
  • completing shift reports, recording and reporting any symptoms or behaviours which may be indicative of a significant physical, psychological or emotional change to his/her Senior Support Worker           
  • planning and organizing job activities to ensure effective implementation of service responsibilities
  • participation in work schedule as assigned
  • performing other requested support as may be assigned by Senior Support Worker

III.       INTRA AGENCY TEAMWORK

(F)   

  • effective, open communication and co-operation with all co-workers to ensure a consistent team approach
  • participation and contribution to meetings, conferences or training events
  • regular reporting to the Senior Support Worker of any concerns on behalf of people receiving support
  • contribution to a positive and productive work environment through such qualities as enthusiasm, adaptability, innovativeness
  • performing all functions in a safe manner, being alert to the health and safety of yourself, coworkers and individuals who receive support
  1. COMMUNITY LIAISON

(G)       Responsibilities include:

  • supporting individuals to maintain a respected place in the community
  • facilitation of positive relationships with parents, advocates, other agencies, and the community in general
  • assisting people to use the wide variety of community resources

SCHEDULE:   Rotating shifts including weekends, as required by needs of people supported. Must be willing to work flexible hours within a variety of settings.
